HOW TO REQUEST COPIES OR AUTHORIZE RELEASE OF YOUR MEDICAL RECORDS

Click on the link below to access the Release of Information form for your medical records. As a patient, you have the right to receive copies of your medical records from Valley Hospital. In order to obtain copies of your records or to authorize release of your medical records to another party, you must complete, sign and date the "Authorization to Use and Disclose Protected Health Information" form. Please see the link below to download this form.

You may fax the completed form to our Health Information Management Department or hand deliver it to the department during business hours, Monday thru Friday, from 8:00 a.m. to 4:30 p.m. The fax number is 509-473-5741. The physical location of the department is inside Valley Hospital, on the 1st floor across from the Imaging Department, at 12606 E. Mission Avenue, Spokane Valley, WA 99216.

Valley Hospital contacts with IOD Incorporated, a professional medical record copying service, to ensure that your copies are available to you as quickly as possible.

Prior to copying your records, IOD Incorporated would like you to know that there is a charge for this service and pre-payment is required.  Prices are as follows:

1 - 9 pages: No charge

10 - 30 pages: $0.88 per page

31+ pages: $0.67 per page

Postage and tax (8.7%) will be charged when applicable.

The ability to charge for copying of medical records, to cover the costs of labor and supplies, is mandated by the Washington State Legislature as outlined in RCW 70.02.

We often recommend that you request only the pertinent parts of your medical record in order to limit the number of pages copies. This usually includes all of the medical reports documented by the physician (e.g. discharge summary, operative reports, consultation reports) relevant progress notes from your visit and diagnostic tests (e.g. Lab, Radiology, EKGs).

IOD Incorporated will contact you with the pre-payment amount. Medical records will not be copied until pre-payment is received. Your copies will be available within three days of payment. You may pay by credit card, cash or check. You can mail payment to IOD Incorporated at the address below or come to Valley Hospital during the business hours listed above to make a payment in person in the Health Information Department.
